When a patient is not responding well to treatment, a behavioral health screening may show they need additional support. Guide the care conversation, starting with the question, “What matters to you?” and determine an appropriate referral. 

If you are unable to provide the support the patient needs in-house, make it as easy as possible for them to get help. Work on building partnerships with community groups that can provide more options for your patients.
