Peer review is a key aspect of professionalism in medicine, promoting quality improvement through analysis and discussion of clinical problems. Hospital medical staffs have long been the primary organ for the peer review process. But as medical groups increasingly provide more complicated care in greater volume, as more physicians practice exclusively outpatient medicine, and as third-party payers demand quality improvement processes, medical groups need to conduct peer review in-house.

Legal concerns about antitrust liability, liability for negative reviews and disciplining physicians, with fairness for all involved, can be mitigated by processes that maximize federal and state legal protections.
