Jennifer Schwartz, LMSW

Jennifer Schwartz

Jennifer Schwartz is a licensed clinical social worker and has worked as a therapist specializing in serving the LGBTQ community, PLWHA, and other marginalized communities since 2012. Additionally, Jennifer is privileged to provide continuing education to other health and mental health professionals on a variety of topics including care coordination, supervision and cultural considerations for providers serving the polyamorous, LGBTQ, BDSM/Kink communities, as well as clients who are Fat/living in larger bodies. Jennifer currently leads the Behavioral Health department at Corner Health Center, in Ypsilanti, Michigan, following a 9-year tenure as a mental health leader within the Detroit community.

Jennifer received her MSW from University of Michigan and her BSW from the University of Michigan-Flint, where she tailored her educational experiences toward sexual health education and sex therapy, as well as general mental health.

Sarah Coles, MD, FAAFP, Program Director, Colorado Plateau Family and Community Medicine Residency at North Country HealthCare

Sarah Coles, MD

Sarah Coles, MD, FAAFP, is the program director for the Family and Community Medicine Residency at North Country HealthCare, a teaching health center in Flagstaff, Arizona. She is also an associate professor in the Department of Family, Community and Preventive Medicine at the University of Arizona College of Medicine–Phoenix and past chair of the American Academy of Family Physicians Commission on Health of the Public and Science. She is practicing as a board-certified family physician.  She obtained her medical degree from the University of Arizona College of Medicine–Phoenix and completed her residency at Banner Good Samaritan Family Medicine Residency. Dr. Coles has expertise in evidence-based medicine, integrated behavioral health care, and population health.  She is actively involved advancing behavioral health integration in primary care through community engagement, institutional collaboration, governmental engagement and advocacy, and leadership.
