Cleveland Clinic becomes first hospital certified as medical home

The Joint Commission has awarded its first Gold Seal of Approval® for Primary Care Medical Home (PCMH) certification for hospitals to the Cleveland Clinic for demonstrating compliance with the group’s national standards for health care quality and safety. 

Cleveland Clinic underwent a rigorous on-site survey in September, during which Joint Commission surveyors evaluated the hospital on how effectively primary care clinicians and interdisciplinary teams work in partnership with patients to provide comprehensive, coordinated and patient-centered care. In total, 39 practices at 29 sites that involve 230 primary care physicians and advanced practice nurses received Primary Care Medical Home certification at the Cleveland Clinic. 

The Joint Commission’s PCMH certification option for hospitals, launched early last year, helps accredited hospitals and critical access hospitals improve the timeliness and appropriateness of treatment, increase patient satisfaction, improve patient outcomes and reduce the overall costs to the health care system. 

The Joint Commission also offers PCMH certification for ambulatory facilities
