Transforming Clinical Practice Initiative

The AMA was part of the Transforming Clinical Practice Initiative (TCPI), a 4-year grant from the Centers for Medicare & Medicaid Services/Center for Medicare & Medicaid Innovation (CMS/CMMI). This initiative provided technical assistance and support to more than 140,000 clinicians in integrating quality and process improvement strategies through clinical coaching and quality improvement assistance, as well as building upon and spreading existing change methodologies and transformation tools.

The AMA was selected to be one of 10 Support and Alignment Networks (SAN) under the TCPI grant. Review the accomplishments (PDF) from the AMA-SAN throughout the grant lifecycle.

As a Support and Alignment Network (SAN), the AMA provided technical assistance to our nation's physicians by improving the current quality of care, increasing patients' access to information and assisting physicians in moving through the phases of practice transformation.

In collaboration with the CMS, the AMA:

  • Promoted the TCPI's goals to the broader health care community
  • Used the AMA STEPS Forward™ practice improvement strategies platform to develop and digitally host educational activities for TCPI participants
  • Accelerated the maturation of clinical data registries
  • Conducted annual collaborative events to elevate the capabilities of Practice Transformation Networks (PTNs) and SANs

The AMA coordinated resources and tools required to continue maturation at a pace consistent with the TCPI goals to accelerate the maturation of clinical data registries, enable practices to initiate quality improvement programs and adopt quality-based payment models.

The AMA also created a Quality Reporting and Qualified Clinical Data Registries (QCDR) resource collection on the STEPS Forward™ site to provide clinicians with the tools they need to become proficient users of QCDRs and assist in preparing clinicians for value-based care models. The AMA sponsored an annual meeting for QCDR stewards under the auspices of the grant (PDF).

AMA put in place a roadmap that incorporates a step-wise approach to assist in practice transformation. The overall series was intended to help practices meet their aims and drivers of practice transformation. These practical webinars provided many useful resources—from both AMA and other partners—to help practices meet the phases of CMS’ Transforming Clinical Practice Initiative. These webinars were folded under the larger Share, Listen, Learn (SL2) umbrella.

To bring more of the resources of the AMA to the TCPI community, the AMA launched a monthly webinar series in 2017. The TCPI: Share, Listen, Learn (SL2) Webinar series made resources available in one place and provided simple yet instructive information on topics that related to TCPI goals. The series helped organizations and practices with health care transformation to value-based care by sharing existing resources, listening and participating in live events, and learning from resources in the AMA Ed Hub.
