AHRQ toolkit aims to improve discharge for diverse populations

Physicians working at hospitals should know about a newly updated resource that can help reduce hospital readmission rates by replicating a discharge process that resulted in 30 percent fewer readmissions and emergency room visits.

Developed by the Agency for Healthcare Research and Quality (AHRQ), the Re-Engineered Discharge Toolkit provides guidance to implement the toolkit for all patients, including those with limited English proficiency and from diverse cultural backgrounds. The toolkit employs health literacy strategies to ensure that patients know how to care for themselves when they get home.
