Aetna Inc. 2002Aetna, based in Hartford, Conn., is the nation's largest health insurer, with operations in all 50 states. Aetna has converted from a general insurance to health insurance only, in part thanks to acquisitions such as U.S. Health care and Prudential's health unit in the Northeast and NYLCare's operations in Texas.
Anthem Inc. 2002Anthem, based in Indianapolis, is one of the nation's largest operators of Blue Cross Blue Shield plans, with a presence in Indiana, Kentucky, Ohio, Connecticut, New Hampshire, Maine, Colorado and Nevada.
CIGNA Health Care 2002CIGNA, based in Philadelphia, is a rarity -- a health insurer that is involved in other kinds of insurance as well. CIGNA is strongest in the Northeast and West. It's the third-largest private health insurer in the nation.
Foundation Health Systems 2002Foundation, based in Woodland Hills, Calif., runs private managed-care networks in the West. Harvard Pilgrim Health Care 2002Harvard Pilgrim, based in Brookline, Mass., is a nonprofit health plan prominent in the Northeast. It pulled out of numerous states because of financial troubles and now operates almost solely in Massachusetts. Health Care Service Corp. 2002HCSC, based in Chicago, owns the Blue Cross Blue Shield plans in Illinois and Texas. Like most Blues plans, it's trying to expand, what with the recent purchases of Blues plans in New Mexico, Washington, Oregon, Utah and Idaho. Highmark 2002Highmark, based in Pittsburgh, runs Blues plans in western Pennsylvania. It is particularly strong in the South.
Kaiser Permanente 2002Kaiser Foundation Health Plans, based in Oakland, Calif., have among the last of the staff-model HMOs. The company runs physician groups, hospitals and health plans in 11 Western, Midwestern and Southeastern states, plus Washing ton, D.C. It's the largest nonprofit HMO in the country.
Oxford Health Plans 2002Oxford, based in Trumbull, Conn., has a strong presence in the Northeast.
PacifiCare Health SystemsPacifiCare, based in Santa Ana, Calif., runs private health plans mostly in the West and Southwest. It jumped into Medicare managed care further than most plans, and as a result the company has had some financial struggles.
The Regence Group 2002The 3 million-member Regence Group, based in Portland, Ore., includes plans in Oregon, Washington, Utah and Idaho.
Trigon Healthcare Inc. 2002Trigon, based in Richmond, Va., runs the Blues plan inVirginia and has operations throughout the Southeast and Mid-Atlantic regions. UnitedHealth Group 2002UnitedHealth Group, based in Minnetonka, Minn., runs its health plans under its UnitedHealthcare subsidiary. Its presence is strongest in the Midwest and South; it is the nation's second largest private health insurer.
WellPoint Health Networks 2002WellPoint, based in Woodland Hills, Calif., runs Blue Cross of California, Blue Cross Blue Shield of Georgia, and various other plans nationwide under its Unicare subsidiary.
