Vaccine Information Statements

The most up-to-date versions of the Vaccine Information Statements (VIS) for the vaccine-preventable diseases can be downloaded from the Immunization Action Coalition's Web site.

Please continue to distribute VISs to your patients prior to vaccination, as required by law. Also be sure to read the following files for information on using the vaccine information materials:
