The AMA Private Sector Advocacy’s (PSA) online Health Plan Complaint Form gives physicians and their staff an opportunity to provide the AMA with critical data on the hassles physicians experience on a day-to-day basis in the managed care environment. This information is critical in helping the AMA identify and combat unfair health insurer and payer business practices. The HPCF is an easy-to-use form with drop-down menus.

Modifications have recently been made to the HPCF that take into account changes in the marketplace, such as recent mergers and acquisitions, as well as the greater level of complexity that has been introduced into the physician practice.

The HPCF asks for limited personal information in order to allow you to access the HPCF. The data will be processed and aggregated in a secure and confidential manner. To ensure confidentiality of your information, please do not include any patient specific information.

Security
