Closing the Referral Loop Project
The Wright Center for Graduate Medical Education, American Medical Association (AMA)-convened Physician Consortium for Performance Improvement® (PCPI®), and Pennsylvania Department of health are hosting our Second Learning Session on Dec. 9, 2014 at The Hilton Scranton and Conference Center for Closing the Referral Loop. We invite all primary care physicians, cardiologists, and staff participating in Closing the Referral Loop. The objectives of this learning session is to discuss data collected, the impact of the CRL work, EMR process changes, and managing referral barriers. We hope you can join!
The aim of the Closing Referral Loop project is to improve processes related to physician to physician referrals by establishing accountability and negotiating responsibility for referrals between specialists and primary care physicians and improving information transfer. The Closing the Referral Loop project will work with a small number of prototype organizations to test interventions and approaches to improve the referral loop process.
