Now Available: First EHR Incentive Program Payments

CMS (7/2011) Eligible professionals can get up to $44,000 by participating in the Medicare Electronic Health Record Incentive Program and up to $63,750 for Medicaid. Eligible professionals and hospitals have already received more than $75 million in first payments from the Medicare EHR Incentive Program. Including the $83.3 million in Medicaid EHR Incentive Program payments, the Federal government has given out more than $158 million since the two programs opened in January. Survey: Publicly insured children receive less comprehensive care than privately insured

HealthDay (7/22, Dotinga) US children with publicly funded health insurance receive "less comprehensive primary care than those who are privately insured," according to study in the journal Academic Pediatrics. Researchers analyzed data from a 2007 national survey of households with children 17 years old and younger. Parents were asked about their child's "usual source of care, familiarity with a personal doctor or nurse, ease of obtaining referrals, access to family-centered care and communication between different" healthcare providers; and, only "45 percent of children with public insurance met all five of those criteria regarding care," compared to 66 of children with private insurance.

Coding Tips: Evaluative and Therapeutic Services

Codes 92601 and 92603 describe post-operative analysis and fitting of previously placed external devices, connection to the cochlear implant, and programming of the stimulator. Codes 92602 and 92604 describe subsequent session for measurements and adjustment of the external transmitter and re-programming of the internal stimulator.


Coding Q&A—Surgery: Evaluation and Management Services

Question: When reporting hospital discharge day management services (99238, 99239), is the final examination of the patient required?

Answer: No. The Hospital Discharge Services guidelines in the CPT codebook state that the hospital discharge day management codes are to be used to report the total duration of time spent by a physician for final hospital discharge of a patient. The codes include, as appropriate, final examination of the patient, discussion of the hospital stay (even if the time spent by the physician on that date is not continuous), instructions for continuing care to all relevant care givers, and preparation of discharge records, prescriptions, and referral forms. The reference to the phrase “as appropriate” has been stated in the CPT guidelines since CPT® 1996 and addresses the circumstance wherein patient examination was either not necessary or not performed but the other element(s) were provided.
