Coding Q&A—Surgery: Digestive System

Question: Our physician is performing an antegrade double-balloon enteroscopy. What is the difference between a retrograde and an antegrade balloon enteroscopy, and is there a different CPT code for each procedure?

Answer: Antegrade enteroscopy (through the mouth) is reported with code 44360, Small intestinal endoscopy, enteroscopy beyond second portion of duodenum, not including ileum; diagnostic, with or without collection of specimen(s) by brushing or washing (separate procedure), if the procedure did not reach the ileum, or code 44376, Small intestinal endoscopy, enteroscopy beyond second portion of duodenum, including ileum; diagnostic, with or without collection of specimen(s) by brushing or washing (separate procedure), if the ileum was visualized.

So, to differentiate, retrograde enteroscopy is performed through the small intestine. No codes exist to describe this procedure. Should the colon be studied, the physician would report code 44799, Unlisted procedure, intestine, to describe the retrograde evaluation of the small intestine.
