Measure specifications
The Physician Consortium for Performance Improvement (Consortium) provides specifications for obtaining the required data elements for its clinical performance measures based on the selected data source.  More detailed specification information for electronic medical records, paper medical records, and use of administrative data is available.

The files above are the initial release required for adoption of Consortium-developed measures within this measure set into electronic health record format.
