Permission for Royalty Free Use in Continuing Medical Education
The American Medical Association (AMA) on behalf of the Physician Consortium for Performance Improvement® (Consortium) grants you and your organization (you) non-exclusive, royalty-free, non-transferable permission to incorporate and distribute in the United States the physician performance measures and related documentation that include the AMA’s copyright notice and are posted on www.physicianconsortium.org (Measures) only in your continuing medical education (CME) programs (Programs) subject to the following agreements:

In consideration of this permission, you will not change the Measures (except for format and stylistic changes necessary to incorporate the Measures in the Programs) or create derivative works of the Measures;

You will include all notices, disclaimers and attribution statements as included in the Measures;

You may not use the name of the AMA, the Consortium or its members in the promotion of your Programs other than to state that the Programs include Measures of the Physician Consortium for Performance Improvement without any statement or implication of endorsement of your Programs;

You acknowledge that the Measures are provided “AS IS” and agree to the disclaimers on the Measures;

Any use not authorized is not permitted and AMA may terminate this permission if you violate its terms; and

This permission shall be governed by Illinois law without regard to choice of law principles and shall remain in effect for three (3) years from your date of acceptance.
