Personal Health Records


Personal health records (PHRs) are designed to provide the most relevant patient information and provide a smooth transition across providers of care. In addition to information provided by the clinician, typically via the EMR, the PHR allows for the patient to input information about their health, such as over-the-counter medication, insulin levels, exercise routines, etc. Without an EMR, PHRs can also be generated via stand-alone products.
