CPT® Pathology & Laboratory Coding Change Request Form

The CPT^® Pathology and Laboratory Coding Change Request Form plays a vital role in maintaining and increasing the efficiency of the CPT process. It can be used to submit a Pathology and Laboratory coding change request for any one of the three categories of CPT codes: Category I CPT Codes, Category II CPT Codes–Performance Measurement, and Category III CPT Codes–Emerging Technology. This form requests changes to Pathology and Laboratory codes only. All other coding change requests should be submitted using the CPT Coding Change Request Form.
