Nominations to AMA Councils/Committees Instructions

1. The sponsoring organizations are requested to determine the candidates' willingness to serve, if selected, prior to submitting nominations to the AMA.

2. Physician members of AMA Councils/Committees must be active members of the AMA at the time of election/appointment and throughout the duration of their term.

3. Nominations for AMA Councils/Committees should be presented on the Council/Committee Nomination Form. (Word, 55KB) Student nominations should be presented on the student application form.

4. All pertinent information must be provided within the space provided on the form. Please complete multiple forms for multiple nominations.

5. The sponsor's statement should be not less than 50 words, nor more than 250 words.

6. Please enclose an executive curriculum vitae (no more than 3 pages) for the candidate with the nomination form.

7. Please review carefully the AMA's Conflict of Interest Policy (Word, 363KB) and Principles (Word, 84KB), which provide explanatory text and examples. If you are successful in attaining the AMA leadership position which you are seeking, you will need to complete and return a conflict of interest disclosure form.

As you carefully review these documents, please also consider if there are pending matters, or matters which you anticipate may occur during your term of office, which could, in your view, reasonably be anticipated to adversely impact your license to practice medicine or your ability to discharge fully the duties you are seeking--without embarrassment to yourself or to the AMA.

If you have questions about these documents, including how to complete the conflict of interest disclosure form (Word, 58KB), the AMA's General Counsel is available to provide guidance. The General Counsel is also available to discuss any other matters you may wish to raise.

8. Confirm that you have reviewed the AMA's Conflict of Interest Policy (Word, 363KB) and Principles (Word, 84KB), and understand the guidance provided above, by signing your nomination form in the space provided.

Please e-mail nominations by the posted deadlines to: brenda.vidana@ama-assn.org
