
Eligibility
Individuals and organizations are eligible to apply for grants which further the charitable and educational purposes of the AMA Foundation. Grants made in 2008 will support programs addressing these four public health issues:
Healthy Lifestyles
To include programs such as nutrition & physical fitness, immunizations, HIV/AIDS education and prevention, skin & other cancer awareness/prevention, teen pregnancy, alcohol, tobacco, & drug prevention/education, smoking cessation and the prevention of favorable depictions of smoking in the media.
Public Safety
To include programs such as anti-bullying, domestic violence, suicide prevention, teen dating violence and internet safety.
Patient Safety & Health Literacy
To include programs such as health literacy projects and projects that aid an organization’s ability to identify and report medical errors, implement proven practices to reduce errors and create provider and consumer awareness of the issue.
You may submit up to 4 pages of supplemental information such as brochures, news articles, etc. The application form must be typed.
Applications must be received at the AMA Foundation Office on or before July 15, 2008. Applications received after this deadline will not be considered.

To better ensure a timely arrival, we recommend you use a courier service such as FedEx, UPS or DHL. The Foundation prefers to receive a hard copy of your application in the mail.
If you need to submit the application via email, you will need to scan your application, including supplemental material, into one PDF, with the required signature.

Instructions for 2007 Fund for Better Health grant recipients
Grant recipients must submit a final report form (Word, 63KB). All forms should be sent to the AMA Foundation office.
