Measure specifications
The Physician Consortium for Performance Improvement (Consortium) provides specifications for obtaining the required data elements for its clinical performance measures based on the selected data source.  More detailed specification information (PDF, 14KB) for electronic medical records, paper medical records, and use of administrative data is available.

Specifications for electronic medical records

• Algorithm for measures calculation to be provided at a later date.
• Release Notes to be provided at a later date.
• Coding specifications (CPT® Category I, CPT® Category II, ICD-9, SNOMED CT®, LOINC®, NDC) to be provided at a later date.

Specifications for paper medical records

• Prospective data abstraction tool to be provided at a later date.
