Commercial Distribution License Application
Vendors interested in incorporating Consortium measures into their electronic health record or other products for quality improvement purposes may request a distribution license agreement.
Distribution licenses are non-exclusive, non-transferable, and royalty-free.
To obtain a license, fax a completed distribution license application (MSWord format) to Intellectual Property Services, American Medical Association, 515 North State Street, Chicago, IL 60610.
Continuing Medical Education (CME) Permissions
Royalty-free permission to reprint and distribute Consortium measures are available to CME providers. Permission is granted through an expedited online CME permission agreement.
