No Contact and No Release Forms
A Do Not Contact restriction on a Physician Masterfile record ensures that the physician's information will not be released for purposes of marketing to the physician via mail, telephone or fax. It also means the physician may no longer receive medically related information from medical publishers, continuing medical education providers and pharmaceutical manufacturers using the AMA Physician Masterfile as a list source.
To add a “Do Not Contact” (DNC) restriction to a Physician Masterfile record, a physician must complete the No Contact Form and submit it to the AMA. Please allow eight (8) weeks for this process to take effect. Use your AMA account username/password to submit the No Contact Form electronically. If you need an AMA account username/password, register now.

A Do Not Release restriction prohibits the AMA from releasing the physician's Physician Masterfile information for any reason. Under this restriction, the physician’s profile is not available to credentialing verification organizations, hospitals or managed care organizations requesting credentials unless the physician provides written permission. In addition, the physician will not receive health hazard warnings, drug recall notices, or medically related information from medical publishers, continuing medical education providers or pharmaceutical manufacturers using the AMA Physician Masterfile as a list source.
To add a “Do Not Release” (DNR) restriction to a Physician Masterfile record, a physician must complete and sign the DNR Authorization Letter and submit it to the AMA via mail or fax.
