Medication Reconciliation

Program description
This patient safety unit provides information for physicians to heighten awareness of the integral role of medication reconciliation in the safe use of medications. The monograph "The Physician's Role in Medication Reconciliation" provides a framework for physicians to understand their role and responsibilities in this often difficult process. To enhance communication and care coordination, the monograph offers general concepts and strategies, implementation steps, and case-based learning. Realizing that patient engagement is essential to safe medication management and the reconciliation process, the program includes a guide to completing an AMA My Medications© patient medication card.

Hardward and software requirements
For this CME program your computer should be Windows 98, Windows NT or Windows 2000 compatible. Internet Explorer 6.0 or Netscape 4.7 (or higher) is also required. If you are using a modem to connect to the Internet, you will need a modem speed of at least 56K. Viewing the PDF will require Adobe® Reader®. As you will be printing your CME certificate online, your computer must be connected to a printer.

To earn credit for this activity, physicians must read the materials and complete the medication reconciliation evaluation form. Physicians may earn credit for this activity from Nov.5, 2008 until Nov. 5, 2010. This activity will no longer be certified after that date.
