CPT codes and descriptions are copyright 2008 American Medical Association. All rights reserved. CPT is a registered trademark of the American Medical Association (“AMA”).

Grant and Limitations. You, as an individual, are authorized to use Current Procedural Terminology (“CPT®”) only as contained in the following publications of CPT Category II codes, entitled Category II Code Section, Category II Medium Descriptors, Appendix H, and Short Descriptor Changes/Additions (the “Files”), solely for your own personal information and only within the United States.

Commercial use of the Files is prohibited under this Agreement.

Any use not authorized herein is prohibited, including by way of illustration and not by way of limitation, making copies of CPT for resale and/or license, transferring copies of CPT to any party, or creating any modified or derivative work of CPT. License to use the Files or CPT for any use not authorized herein must be obtained through the AMA, Intellectual Property Services, 515 N. State Street, Chicago, IL 60654. Applications are available at the AMA Web site, http://www.ama-assn.org/go/cpt.

U.S. Government Rights. The Files include CPT which is commercial technical data and/or computer data bases and/or commercial computer software and/or commercial computer software documentation, as applicable which were developed exclusively at private expense by the American Medical Association, 515 North State Street, Chicago, Illinois, 60654. Users of Performance Measures should refer to Web sites referenced in the Files to obtain complete Performance Measures and specifications as well as to obtain any necessary permissions to use these Performance Measures. The AMA disclaims responsibility for any consequences or liability attributable to or related to any use, non-use, or interpretation of information contained or not contained in these Files.

Termination. This Agreement will terminate if you violate its terms or if AMA no longer makes the Files available. You agree that AMA will be awarded court costs and reasonable attorney fees if it prevails in any action or proceeding against you due to a breach of this License. You agree upon termination to destroy any copies of the Files.

General. This Agreement will be governed by the laws of the State of Illinois without regard to choice of law principles. You acknowledge that you have read this Agreement, understand it and agree to be bound by its terms and conditions. You further agree that this is the only agreement between us relating to the subject matter of this Agreement. If any provision hereof is held to be unenforceable or contrary to public policy the parties agree that the remaining portions are enforceable.

This Agreement shall be effective upon your acceptance by choosing the “Accept” button.
